Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 7/2/2020 and concluded on 7/6/2020. The Principal was contacted by telephone to initiate the inspection. There were 56 children present with 14 staff. The inspector emailed the Principal a list of items required to complete the inspection. The inspector reviewed 4 children?s records and 3 staff records submitted by the facility to ensure documentation was complete.
The information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the facility.
The licensing inspector has reviewed with the Provider COVID-19 Essential Guidance for Child Care programs.
Based on record review and interview, each child does not have documentation of a physical exam within the first 30 days of attendance. Evidence: Child #2 (date of enrollment: 8/19/2019) file does not contain a physical exam. The Principal stated that they had obtained a physical but it is not in the child's file and can not be located.
Plan of Correction:
A new physical will be obtained as soon as possible.

Description:
Based on record review and interview, staff did not resubmit the results of a follow-up tuberculosis (TB) screening every two years. Evidence: Staff #3 most recent TB screening is dated 1/28/2018. The center Principal confirmed that a follow-up TB screening hadn't been completed yet.
Plan of Correction:
The Principal notified the staff member during the inspection. I'll get completed within the week.
