An unannounced monitoring inspection was conducted on March 12, 2020 from approximately 9:05am-11:30am. Upon arrival to the facility, there were 36 children present with 11 staff members. Additional staff were on site for administrative and support purposes. The center has a total enrollment of 92 children. The center offers before and after care services for school age children. The children were observed during circle time, doing worksheets, coloring and playing freely. Infants were observed feeding, napping and being diapered. Six classrooms, an open indoor play space and the outdoor area were inspected. The center does not offer transportation.
Based on record review, in five of eight staff records reviewed, the center did not ensure that staff have results of a search of the Central Registry within 30 days of employment.
Evidence: The Director confirmed that there were not Central Registry results for the following staff members: Staff #1-date of hire 8/30/19 Staff #3 and Staff #6-date of hire 1/27/2020 Staff #4-date of hire 1/23/2020 Staff #7-date of hire 2/6/2020
Plan of Correction:
We will get the forms filled out today from those teachers and forward the results to the licensing inspector once they are received.

Description:
Based on observation and staff interview, the center had not complied with provisions of this section by disclosing all required information in writing to the parents and guardians of the children in the center.
Evidence: The Director confirmed that the center had not posted the fact that it is exempt from licensure in a visible location on the premises.

Director will type up that the center is religiously exempt and excluded from licensure and will post on the board in the hallway.

Description:
Based on record review, in four of eight staff records reviewed, the center did not ensure that each person in a supervisory position has been certified by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under his supervision.
Evidence: The Director confirmed that there was no documentation that Staff #3 (date of hire 1/27/2020), Staff #4 (date of hire 1/23/2020), Staff #5 (date of hire 2/17/2020) and Staff #6 (date of hire 1/27/2020) had a completed staff health report on file.
Plan of Correction:
Director will follow up with each staff person today and have those done.

Description:
Based on staff interview, the center had not established and implemented procedures regarding reports of serious injury or death of children while attending the child care center.
Evidence: The Director confirmed that the center had not established the following procedures:
1. Ensuring that all incidents involving serious physical injury to or death of children attending the child day center are reported to the Commissioner.
2. Reports of serious physical injuries, which shall include any physical injuries that require an emergency referral to an offsite health care professional or treatment in a hospital, shall be submitted annually. 3. Reports of deaths shall be submitted no later than one business day after the death occurred.
Plan of Correction:
Director will type up a step by step plan. A copy of the forms will be completed and placed in a binder. Each staff will be trained in these procedures.

Description:
Based on record review, in one of eight staff records reviewed, the center did not obtain a copy of the results of a search of the central registry maintained pursuant to ? 63.2-1515 and any child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years.
Evidence: Staff #3 (date of hire 1/27/2020) indicated on the sworn statement as to living outside of the State of Virginia within the last five years and the center had not conducted an out of state child abuse and neglect search for the state in which she previously resided.
Plan of Correction:
We will get the forms filled out today from those teachers and forward the results to the licensing inspector once they are received.
