22.1 Early Childhood Care and Education

Technical Assistance:
Discussed medication log documentation, allergy list and action plans and injury documentation. Recommended to look at the model forms.

Comments:
A subsidy health and safety (SHSI) inspection was completed on August 10, 2023 from 12:15pm - 3:05pm . There were 37 children and 8 staff and additional administrative staff was present. During the inspection children were observed napping , freely playing, story time qnr transitoning from nap time. Five children records were reviewed and five staff records were reviewed.

Information gathered during the inspection determined non-compliance(s) with applicable requirements and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-790-550-2-a
Description: Based on record review and staff interview, the vendor did not ensure staff had Virginia Child Protective Services Central Registry check 30days after employment.

Evidence:
During the inspection on August 10, 2023 the center was not able to provide Central Registry results for Staff # 2 (hire date was 08/19/2022).

Plan of Correction: Central Registry/Out of State Registry: I have submitted requests for those missing this check. Employee from out of state is working on getting his notarized and working with obtaining his records.

Standard #: 8VAC20-790-550-2-b
Description: Based on record review and staff interview, the vendor did not ensure a out of state child abise and neglect search was requested and/or follow up on out of state Child abuse neglect request was completed 45 days after request was submitted.

Evidence: Staff #1 hire date was 02/02/2023. Staff #1 lived out of the state of Virginia in the past 5 years. During the inspection the center was unable to determine if a out of state centeral registry was submitted to the state he lived in the past 5 years.

Plan of Correction: Central Registry/Out of State Registry: I have submitted requests for those missing this check. Employee from out of state is working on getting his notarized and working with obtaining his records.

Standard #: 8VAC20-790-630-A
Description: Based on observation, the vendor did not ensure that areas ogf equipment of the enter insode and outside, is clean, safe and operable.

Evidence: In the gym, there was cracked window on the door, rust in the boys bathroom and light was not working in the bathroom . Staff confirmed the bathroom is used by the program.

Plan of Correction: A glass company has been contacted to repair the cracked windows

Standard #: 8VAC20-790-630-C-2
Description: Based on observation and staff interview, the vendor did nto ensure electrical outlets have protective covers.

Evidence:
In the infant room, there was two outlets not covered . The outlets were shown to staff in charge during the inspection.

Plan of Correction: The adapter that was missing outlet covers has been removed and covers placed in the socket. We also have installed the child safety outlets that close automatically.

Standard #: 8VAC20-790-640-A
Description: Based on observation and staff interview, the vendor did not ensure hazordous substances shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:
In the following areas there was hazordous substance that were not kept locked:

1-In the hallway, there was hand santizer and soap both had two warning labels keep out of reach of children, warning, caution and/or flammable.
2-In the 4 year old room, there was soap labeled with keep out of reach of children and warning.
3-In the 3 year old room, there was soap labeled with labeled with keep out of reach of children and warning.

Plan of Correction: All hazardous material has been removed from the classroom or locked in a cabinet. We also installed automatic soap dispensers.

Standard #: 8VAC20-790-790-C
Description: Based on observation and staff interview, the vendor did not ensure Emergency evacuation and shelter in place procedures or maps shall be posted in a location conspicuous to staff and children on each
floor of each building.

Evidence:
There was a map posted for evacuation but did not have posted procedures or map for shelpter in place.

Plan of Correction: I am working on developing a shelter in place map/procedure to be placed in the room. We have updated the escape route map in each room, office and center exits.

Standard #: 8VAC20-790-790-D
Description: Based on staff interview and observation, the vendor did not ensure a 911 or local dial number for police, fire, and emergency medical services and the number of the regional poison control center shall be posted in a visible and conspicuous place.

Evidence:
Staff was unable to locate the list of emergency numbers.

Plan of Correction: Emergency numbers have been posted in the center office

Standard #: 8VAC20-790-800-B
Description: Based on staff interview and record review, the vendor did not ensure to maintain record of dates of the shelter in place and lockdown drill was practiced.

Evidence:
There was not a record maintained of shelter in place and lockdown drills for 2022.

Plan of Correction: Emergency procedures for shelter in place and lockdown were updated in the employee handbook. I have scheduled a drill in September for shelter in place/lockdown with a log sheet kept with our fire drill log.
