Comments:
A self report inspection was initiated on 06/07/2023 and completed 08/10/2023. A self report was made in the areas of behavorial guidance. The evidence gathered during the investigation supported the there was non-compliance with requirements not related to the self report. Violation was issued.
Based on record review and staff interview, the provider did not ensure by staff 30th day of employment there should be Central Registry results . Evidence: Staff #1 did not have a Virgina Central Registry results documentation in the record . The center was unable to locate the Central Registry Check on June 7, 2023 and August 10, 2023.
Plan of Correction:
New management has been implemented . New protocol has been put in place to void missing background checks.
