Comments:
An inspection in response to a self-reported incident was initiated on December 20, 2023 and concluded on January 26, 2024. The report received included information of a staff member using inappropriate behavioral guidance techniques and forbidden action. The Centers staff interviews were conducted, observations and were conducted in response to the self-reported incident.
The evidence gathered during the investigation supported the self-report of non-compliance with standards or law, and violations were issued. Any violations not related to the self-report but identified during the course of the investigation can also be found on the violation notice.
REPEAT Based on staff interview, behavioral guidance shall be constructive in nature and age and stage appropriate and shall be intended to redirect children to appropriate behavior and resolve conflicts. Evidence:
1. Staff #1 confirmed in a written statement and interview on December 4, 2023, that child #1 (3 years old) was knocking over stacked chairs, throwing a chair and toys across the room, and hitting the staff. Staff #1 stated she grabbed child #1 around the waist and sat child #1 down with cross legs. Child #1 was squirming around and refused to calm down and began to struggle. Staff #1 released child #1.
2. Staff #1 confirmed on written statement and interview, child #1 he was throwing shoes, kicking equipment and knocking over office chairs. Child #1 reportedly ran away from the staff . Staff #1 put child #1 in a multi child stroller and strapped into seat until a parent arrived.
Plan of Correction:
We sat down with parent we came up with a plan to use a transition timer. So the child knows when things are going to change.
We started a behavior chart so he can earn stickers and recieve rewards. If child starts to escalate parent will be called.
And if he escalates the staff will have child removed from the classroom and the child will go to safe place.
The staff have been given directions to not restrain children and to not use stroller for behavior redirection.
Center has requested training from local agency for behavioral education. Scheduled a Professional Develoment day in February for training. The inspector will review subsidy requirements during the training.

Based on review of documentation and staff interview, the vendor did not ensure the following actions or threats thereof are forbidden physical punishment.
Evidence: Staff #1 wrapped both of her arms around child #1 (3 years old) waist and sat down cross legged. As child #1 refuse to calm down and began to struggle against staff #1?s arm staff #1 released him.
Staff #1 strapped child #1 in the stroller until a parent arrived to pick up the child.
