Corrections to background check violations must be initiated within 10 days from the date of this inspection.
Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated and concluded on 2/16/2021. The provider was contacted by telephone to initiate the inspection. There were 0 children present. The inspector reviewed with the provider a list of items required to complete the inspection. The Inspector reviewed 3 provider/adult household member records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based on a review of background checks, it was determined that the provider did not obtain a signed Sworn Disclosure Statement for one household resident within 30 days of that person turning 18 years of age. Evidence: Household resident C turned 18 years of age more than 30 days ago. The provider has not asked this household resident to sign a Sworn Disclosure Statement.

I will have her sign the form today, and send a copy to my Inspector.

Based on a review of background checks, it was determined that one adult household resident has not submitted to a fingerprint background check. Evidence: Household resident C turned 18 years of age more than 30 days ago. This person has not completed a fingerprint background check.
Plan of Correction:
She will make her appointment this week. I will send the results to my Inspector.
