Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A modification inspection was initiated on June 12, 2020 and concluded on June 12, 2020. The provider was contacted by telephone to initiate the inspection, in response to license modification request for a change of location. There was 1 child present and 1 staff.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.
Based on observation on June 12, 2020, the provider did not ensure to have a landline telephone, excluding a cordless or cell phone, available, operable, and accessible. Evidence: When asked to see the landline phone, the provider stated it was in a drawer, not hooked-up, and that she couldn't get to it and didn't know where it was.
Plan of Correction:
I have replaced my cordless phone with the landline phone.
