Modification to reflects the new address as: 41921 Visionary Court, Aldie, Virginia 20105. The hours of operation will be: Monday - Friday from 7 am to 5 pm. The capacity will remain set at 4.
Comments:
A VR-FDH application was received by OCCHS (Fairfax) for a change of address for this home. A NMI was conducted at the new address by licensing staff and concluded on 06/21/2023, between the hours of 10 am to 11:30 am. Because the Provider just moved to this location, there were no children in care. The inspector reviewed compliance in the areas as indicated on the Voluntary Registration Health and Safety Checklist: Emergencies; Accident and injury prevention; Health and safety; Child Development; and Laws & Regulations. A total of 4 children records, 1 staff record, and 3 adult household records were reviewed.
Information gathered during the inspection determined that there were (2) non-compliance with the applicable requirements. These violations were documented on the Violation Notice issued to the home.

Based on review, 1 of 4 children files indicated that the Provider did not have proof of adequate immunization received on the date of admission (or prior to admission) for each child admitted.
Evidence - 1. On the date of inspection (06/21/2023), an immunization page was not available for review for Child #2. 2. The start date for Child #2 was listed as 11/29/2023. 3. On the section of the health form that was on file for Child #2, under immunizations, "see attached form" was written. However, no attachment on file was included.
Plan of Correction:
A copy of the immunization page for Child #2 will be obtained and placed on file.

Description:
Based on observation and interview, a protective barrier (safety gate) was not in place at the top of outside stairway that is accessible to children.
Evidence - 1. On the date of inspection (06/21/2023), the is a stairway (6 concrete steps) that lead from the backyard down into the VR-FDH. 2. The Provider stated (during an interview conducted on the date of inspection) that the families will enter the VR-FDH down these steps and through this back entrance. While the children are in the backyard they will have direct access to this stairway. 3. Currently there are 2 children enrolled in the VR-FDH that are under 2 years of age.
Plan of Correction:
A mounted safety gate will be installed at the top of this stairway to prevent direct access by a child.
