Technical Assistance:
Discussion of Renewal Certificate of Registration.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection of the Certificate of Registration for this Voluntarily Registered Family Day Home (VR-FDH) was initiated and concluded on 01/07/2021. The provider was contacted by telephone to initiate the inspection. There was (1) child present (2) points, with (1) caregiver. The inspector informed the provider of a list of items required in order to complete the inspection. The Inspector reviewed (1) child?s record, (3) household records, and (1) staff record submitted by the VR-FDH to ensure documentation was complete.

Information gathered during the inspection determined non-compliance in (1) applicable area and was documented on the violation notice issued to the VR-FDH.

Standard #: 22VAC40-180-100-C
Description: Based on review, the file of Child #1 indicated that not all of the required information was contained within this record.

Evidence -
1. On the date of inspection (01/07/2021), the name, address and telephone number of one or more persons designated by the parent or parents to be called in case of emergency when a parent cannot be reached during the hours the child is in care was not available for review.
2. The only emergency contacts that were listed for Child #1 both on the Health Form, and the Medical Authorization form were Child #1's parents.

Plan of Correction: The Parent shall be contacted at the time of pick up and an emergency contact other than either parent of Child #1 shall be obtained.
