Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review. A monitoring inspection was conducted on 8/12/21. The provider was contacted by telephone and a virtual inspection was conducted. There were 5 children present with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 4 staff and household member records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, one staff record did not contain all required information. Evidence: Staff 2's record contains records and background checks that were completed after the listed hire date of 7/9/21. Reference 1 is dated 8/12/21, Reference 2 is dated 7/22/21. The sworn statement or affirmation is dated 7/12/21, and the criminal record check is dated 7/15/21.
