Comments:
An unannounced monitoring inspection was conducted today, November 9, 2023. The inspector was on site from approximately 1:00 pm until 2:05 pm. There were eight children present with two caregivers supervising for a value of 23 points. Children were observed having lunch. The provider read a story during lunch to the children. Later the children played with cars and prepared for nap. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of seven child records and two caregiver records were reviewed. The inspectors interacted with the caregivers and children in care. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of eight children?s records, the provider did not obtain documentation of a physical examination by or under the director of a physician prior to the child?s attendance or within 30 days after the first day of attendance.
Evidence:
The record of child #1, enrolled 08/21/2023, did not contain documentation of a physical exceeding 30 days from the first day of attendance.
Plan of Correction:
Per the provider: The parent has an appointment next Friday. I will have her bring me a copy of the physical for the record.
