Comments:
A monitoring inspection was initiated and concluded on 4/20/2023. There were 50 children present with 8 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 7 staff records were reviewed. The inspection started at 11:30 am and concluded at 3:45 pm. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on a review of records and interview, the licensee did not ensure that the results of a Federal Finger print based Criminal History Records Search are obtained prior to the employment of staff.
Evidence: The record for Staff #1 documented a hire date of 11/4/2022. The Criminal Records results for Staff #1 were dated 11/7/2022.
The record for Staff #5 documented a hire date of 9/20/2022. The Criminal Records results for Staff #1 were dated 11/14/2022.
The record for Staff #6 documented a hire date of 9/12/2022. The Criminal Records results for Staff #1 were dated 9/19/2022.
The record for Staff #1 documented the date of completing New Employee Orientation as 4/14/2023. The Criminal Records results for Staff #1 were dated 11/7/2022.
Plan of Correction:
The Program Director states that, effective immediately, she will ensure that the results of the Criminal Record History Search have been received before she hires staff and/or begins their orientation training.

Description:
Based on observation and interview, the licensee did not always ensure that the ratio of 1 staff to 8 children was maintained for Two-Year-Old Children.
Evidence: At the time of arrival, approximately 11:30 am, the licensing inspector observed 12 two-year old children with one staff (staff #7). Staff #7 confirmed that all of the children present in the group were two years old. At approximately 11:55 am, a second staff arrived at the center to assist with the care of the two year old children.
Plan of Correction:
The program director stated that a staff in the two-year-old room had to leave that morning due to illness and an infant teacher was able to go in the two-year-old room at that time. However, the infant staff later had to return to the infant room to meet ratios there. The Program Director states that she was working in ratio at the other center location and came to assist as soon as she was able. The program director states that she has been constantly interviewing and hiring additional staff and has more interviews scheduled for this week. An additional program director has been offered a position to begin the first week of May so as to allow her the current PD the ability to oversee operations and fill in ratios as needed.
