Comments:
An unannounced renewal study was conducted today. There are 12 children (33 points) with 2 caregivers present.
4 adult and 5 children's records were reviewed. See violation notice form missing items. Children were observed washing hands, eating lunch and playing with a variety of toys and games. The dog's rabies vaccination is current. Emergency evacuation drills, Shelter-in-place drills, first aid supplies and the menu are all in compliance. Based on direct observation, two seats used by children do not have safety straps in place, as intended by the manufacturer.
Evidence:
Two children were in seats that have an opening for safety straps; however no straps are on the chairs.
Plan of Correction:
I have the belts and install them on the seats.

Description:
Based on a review of records, two caregivers did not obtain a subsequent screening results.
Evidence:
Two do not have documentation of a subsequent TB screening, obtained every two years.
Caregiver A - TB screening is dated 1/26/17 (expired).
Caregiver B - TB screening is dated 1/25/17 (expired).
Plan of Correction:
They will have screening done right away.

Description:
Based on a review of records, the provider did not ensure that the allowable 16 points per adult was maintained.
Evidence:
At LI arrival, there were 12 children present (33 points) with two caregivers.
Plan of Correction:
I miscounted the points for the children's ages. Two children are leaving, and one child has a birthday soon reducing the points.
