Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on April 23, 2021 and concluded on April 26, 2021. The provider was contacted by video call to initiate the inspection. There were 2 children present and 1 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 1 staff record submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based on review of two children's records, two do not contain documentation of a physical examination from a physician prior to the first day of attendance. Evidence: The record for child #1 and #2 do not contain documentation of a physical examination from a physician before the first day of attendance.
Plan of Correction:
I will get a copy of a child's physical before a child starts and put in their file.
