Comments:
The Licensing Inspector for Garden of Prayer #7 Great Beginnings Day Care conducted an unannounced RECDC Monitoring Inspection on 03/09/2020. Licensing Inspector arrived & started inspection at 9 am. There a total of 18 children ages 14 months? 5 years old present under the care of five (5) staff members. The Licensing Inspector reviewed information for five (5) enrolled children & information for five (5) staff members. Center does not administer medication and provides transportation for school-age children. Violations found during today?s inspection are identified on the violation notice. LI Kimbrough conducted an exit interview along with reviewing the Risk Assessment and the areas of non-compliance with the Center Director on March 09, 2020. Based on review of staff records the facility failed to maintain compliance with this standard as required related to Sworn Disclosure Statements.
Evidence: Staff #4, hire date 3/3/20, did not have a Sworn Disclosure Statement as required. This staff was observed taking the children to the restroom during this inspection.
Plan of Correction:
The staff member will fill out the statement today. In the future, upon hire, staff will complete the form.

Evidence: The Exempt Status letter generated from the DSS was not posted as required.

A copy of the letter from the DSS will be posted as required and updated annually for complaince.
