Comments:
An unannounced monitoring inspection was conducted on Thursday, February 20, 2020 to determine the program?s compliance with licensing standards. The licensing inspector was on site from approximately 3:15 PM until 6:00 PM. The licensing inspector observed a total of 70 children being supervised by eight staff members. The required staff-to-child ratios were being maintained. Children were observed arriving to the program, washing hands and taking bathroom breaks, eating snack, playing with magnet blocks, working puzzles, creating with Floam, building with blocks, measuring, drawing and playing with dollhouses. All areas used by children were inspected to include the classrooms, gym, cafeteria, hallways, and bathrooms. The outdoor play areas were not inspected due to inclement weather.

The following information was reviewed during the inspection: daily attendance, injury reports, emergency drill logs, emergency supplies, parent agreements, required postings and menu. Medication and medication authorization forms were reviewed. During the inspection, seven children's records and seven staff records were reviewed.

Description: Based on review of seven children's records and interview, the center did not ensure one child had a physical examination by or under the director of a physician before the child's attendance or within one month after attendance.


Evidence:
The record for Child #6 (first day of attendance 09/03/19) did not contain documentation of a physical exam. A member of management confirmed this during the inspection.

Plan of Correction: The parents were contacted while the licensing inspector was on site and asked to submit a copy of the required physical exam by 02/24/2020.
