Comments:
An unannounced renewal study was conducted from 10:00 a.m. - 12:15 p.m. There were 16 children in care being supervised by 3 staff members. The children were observed participating in structured center play, table top activities, infant feedings and carpet play. Lunch is provided from home. Four staff records were reviewed and in compliance. Six children's records are reviewed and in compliance. There are reportedly no medications being administered to children at this time. The most recent emergency escape drill was conducted 09/30/2019 and the most recent shelter in place drill was conducted 07/26/2019. Based on record review and interview it was determined that the center did not receive annual approval from the health department. Evidence: The most recent inspection from the health department is dated 03/27/2017.

Description:
Based on observation of the pre-school room it was determined that not all hazardous substances were kept locked. Evidence: A container of cleaning agent was observed in the unlocked closet of the pre-school room accessible to children in care.
