Comments:
An unannounced complaint inspection was initiated with an onsite visit on 04/8/2024 in response to allegations received by the licensing office on 04/03/2024 in the areas of Staffing and Supervision. The inspection concluded with phone interviews and electronic document review on 04/10/2024. A total of 1 child?s record, and 1 staff record was reviewed. The inspector arrived for the inspection at approximately 9:45 am and departed at approximately 12:15 pm. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
The information gathered pursuant to an investigation of the complaint supports the allegation and the complaint was determined to be valid.
Complaint related Based on interviews with staff and document review, the center failed to ensure that children under 10 years of age always shall be within actual sight and sound supervision of staff.
Evidence: On 04/02/2024, Child 1 (age 3) left the group of children being taken to the bathroom and was out of actual sight and sound supervision for approximately 2-3 minutes. Child 1 was located approximately 10 feet from the exit of the building.
Plan of Correction:
-Update policies and procedures in the handbook. -Maintenance will install door alarms -Enlist support staff to provide assistance to teachers as needed. -On a daily basis, classroom staff will remind children of classroom rules and the importance of staying with the group.
