Comments:
A mandated monitoring inspection was conducted on May 30, 2023 from 10:40 A.M.-2:30 P.M. There were 81 children present, ranging in ages from six months to five years of age, with 17 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of ten child records and 13 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation, the center failed to ensure that all areas inside the building shall be maintained in a safe condition.

Evidence:
In the Early Step Prep #1 Classroom there was peeling paint on the bottom half of the wall behind the infant feeding table.

Plan of Correction: Spot will be patched and repainted. Tee Spot will apply plexiglass sheet for a permanent fix.

Standard #: 8VAC20-780-500-B
Description: Based on observation on , the center failed to ensure disposable diapers shall be disposed in a leak-proof or plastic lined storage system that is foot-operated or used in such a way that neither the staff member's hand nor soiled diaper touches an exterior surface during disposal.

Evidence:
Staff #1 was observed, after diapering, disposing of a soiled diaper in the trash can. Staff #1 did not use the foot pedal, but manually opened the lid of the trash can with her hand.

Plan of Correction: Documentation log was filled out and discussed immediately after the inspection. Observations of diapering procedures will be added to the weekly operational checklist.
