Comments:
An unannounced inspection was conducted on 6/30/2022, in conjunction with the local department of social services, to investigate a complaint regarding Forbidden Actions and Procedures for Emergencies. Interviews were conducted and documentation was reviewed. It was determined that the portion of the complaint regarding Forbidden Actions was NOT VALID. The portion of the complaint regarding Procedures for Emergencies was determined to be VALID. Other violations not related to the complaint are documented on the violation notice issued to the facility. An exit interview was conducted on 7/22/2022. Based on review of documentation, the facility failed to properly document that two references as to character and reputation as well as competency were checked before employment. Evidence: two references in the record for staff #1 did not identify the full name of the person contacted or who the reference was for.

Description:
Based on interviews and a review of documentation, the facility failed to ensure at least one staff member who meets the qualifications of program leader or director is regularly present in each grouping of children. Evidence: interviews revealed that staff #1 supervises approximately five children in the toddler room daily from approximately 2:30 ? 6:00pm. The record for staff #1 indicated that they are an assistant and they do not have documentation that they have lead teacher qualifications.
