Comments:
A self-reported incident inspection was initiated on 6/6/24 and concluded on 6/6/24. The inspector reviewed compliance in the areas listed above.
This inspection included: * document review * interviews
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Cosmetics, medications, or other harmful agents shall not be stored in areas, purses or pockets that are accessible to children. On June 6, 2024 the keys belonging to staff #1 were on the table in the classroom accessible to the children in care. The keys had pepper spray attached. Child #1, 7 years of age, picked up the keys and accidentally sprayed child #2, 8 years of age, in the face.
Plan of Correction:
Staff items will be in the back office and not on the classroom floor. A verbal training was done that evening to staff. A staff meeting is scheduled for Monday June 17th as well.

Description:
When staff are supervising children, they shall always ensure their care, protection, and guidance. On June 6, 2024 the keys belonging to staff #1 were on the table in the classroom accessible to the children in care. The keys had pepper spray attached. Child #1, 7 years of age, picked up the keys and accidentally sprayed child #2, 8 years of age, in the face. Child #2 required first aid intervention to relieve the affects of being pepper sprayed in the face. Staff #1 did not ensure the care, protection, and guidance of the children by leaving the keys, with pepper spray attached, accessible to the children within the classroom.
Plan of Correction:
Director and staff will ensure that children in their care will be cared and protected from harm. A staff meeting is scheduled for Monday June 17th as well.
