Comments:
An unannounced Code Compliance inspection was conducted with the director and one staff member on July 27, 2023, beginning at 11:20am and ending at 12:38pm. There were 9 children, ages 2 years through 5 years old, in care of two staff members during the inspection. The children were observed watching an age-appropriate show while washing hands to prepare for lunch. The children were also observed eating lunch which consisted of chicken patty sandwich, green beans, fruit cocktail, and milk. Four children's records were reviewed. Three staff records were reviewed. There were no medications on site to be reviewed. Facility policies and procedures were reviewed and in compliance.

Based on observation, the facility staff failed to maintain the appropriate ratio of staff to children. Evidence: The mixed age group of children consisted of 9 children, ages 2 years through 5 years old. The staff to child ratio should be 1:8. The 9 children were in the direct care of 1 staff during the inspection while the other teacher was cooking lunch.

The facility staff will create a way to ensure that the appropriate staff to child ratio is being maintained while preparing lunch.

Description:
Based on a review of documentation, the facility staff did not ensure that all staff members had a completed annual ?Staff Health Report? on file. Evidence: Staff A was missing documentation of a staff health report in their record.
Plan of Correction:
The staff has submitted the document to their physician but has not received the completed document back yet. It will be placed in their record upon receipt.

Description:
Based on a review of documentation, the facility staff failed to obtain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations prior to the first day of attendance. Evidence: Child A and Child B did not have documentation of immunization in their records.
Plan of Correction:
The facility staff has requested the missing documentation from the parent and it will be placed in the children's records.
