Comments:
An unannounced inspection was conducted on March 2, 2023 from approximately 11:15 am to 2:00 pm in response to a complaint that was received on 2/8/23 in reference to emergency evacuation procedures. Interviews were conducted and documentation was reviewed. Follow up interviews were conducted over the phone on 03/10/23. The preponderance of evidence gathered supports violation of standards.

Violations were cited as a result of this inspection.

Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today, 03/13/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Complaint related: Yes
Description: Based on documentation review, and interview, the center failed to ensure that the center?s own policies and procedures are followed. Evidence: Documentation review and interview revealed that an evacuation drill occurred on 12/16/22 at 10:00 am. Interviews revealed that a 6-month-old infant was left alone in the building for approximately 1 minute. The center did not follow its own fire drill policy that states that the ?last teacher out should make sure that no children are left in the room.? Interview on 3/10/23 with staff #1, date of employment 1/4/18, and the Assistant Director confirmed that the center?s policy was not followed.

Plan of Correction: Per the center "both employees were retrained on proper fire drill evacuation procedures on 12/16/22 by Administration. All staff will be retrained on proper fire drill evacuation procedures on 3/21/23."

Complaint related: Yes
Description: Based on interview, and documentation review, the center failed to ensure that children under 10 years of age shall always be within actual sight and sound supervision of staff. Evidence: Documentation review and interviews revealed that an evacuation drill occurred on 12/16/22 at 10:00 am and classroom #1 had 7 children and 2 staff at that time. Interviews revealed that a 6-month-old infant was left alone in the building for approximately 1 minute and was found by the Assistant Director while doing a classroom sweep.

Plan of Correction: Per the center "both employees were retrained on proper fire drill evacuation procedures on 12/16/22 by Administration. All staff will be retrained on proper fire drill evacuation procedures on 3/21/23."
