Comments:
An unannounced monitoring inspection was conducted on 12/2/19 from 1:40pm - 3:00pm. During the inspection there were 38 children ages three years old through five years old in care with 10 staff. Children were observed participating in various activities in the classroom, eating snack, and during rest period. Records were reviewed for five children and nine staff. Medication, emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-510-G
Description: Based on a review of the medication being stored at the facility, it was determined that the facility did not ensure that in order to administer prescription medication, the center has obtained written authorization from a parent or guardian.

Evidence:
1. There was no written authorization for the medication (Benadryl) for child #6.
2. Staff #10 (Program Director) confirmed that there was medication at the facility that did not have written authorization from the parent to be given.

Plan of Correction: The facility responded: There was previously a written authorization for the medication for child #6, but we were unable to find it during the inspection as it may have been removed when the child's other medication was picked up by the parent. The parent of child #6 will be asked to complete a new medication authorization. No medication will be accepted without a written medication authorization.
