Technical Assistance:
Documentation of program leader qualifications was discussed with the program director. A sample qualification checklist to which verification of qualifications can be attached was provided.
Comments:
An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 1:00 PM and departed at 2:50 PM. Thirteen children, ages two years and older were in care with two staff. Nap time was observed. A sample size of five staff records and five children's records were reviewed. It was reported that no medication has been administered to children in care. Injury records were reviewed.
The violation cited was reviewed with the program director prior to the after school pick-up. The inspection notices were reviewed with staff prior to closure of the inspection.
Based upon observation and staff interview, the facility has not ensured that each child's records includes the names, street addresses and telephone numbers for two designated persons to be contact in an emergency if a parent cannot be reached. Evidence: 1. In the record provided for child 3, there are not complete street addresses for the two designated persons to be contacted in an emergency if a parent cannot be reached. 2. In the record provided for child 5, there is only one designated person to be contacted in an emergency if a parent cannot be reached. 3. Staff 1 verified the missing information in the children's records noted above.

The facility responded with the following All required information was obtained during the inspection. In the future, we will ensure that all required information is included in each child's record.
