Technical Assistance:
Discussed with the Assistant Director replacing the locks in the classrooms with a different shaped lock that would be more secure.

Comments:
An unannounced monitoring inspection was conducted today from 9:00am to 11:15am with the Director and Assistant Director. There were 31 children in care, ranging from two to five years of age, supervised by five staff. The children were observed playing freeze dance, reading a book and discussing the new theme, playing in centers, singing, and doing an art project about squares. Diaper changing was also observed. Five children?s records and four staff records were reviewed. The Center has seven staff certified in CPR, First Aid, and Daily Health Observation. Two medications and authorization forms were reviewed and the center has two staff current in Medication Administration Training (MAT). The attendance and emergency drill log were reviewed and were in compliance. The last evacuation drill was documented as 10/15/19, and the last shelter-in-place was documented as 5/30/19. The first aid kit was observed and in compliance. Lunch today is fish sticks, mixed veggies, fresh apples, and milk. Description: Based on observation, not all areas and equipment of the center were maintained in a safe condition. Evidence: On the large playground for the children three years and older, the wooden climbing equipment has splintering, cracked, and deteriorating wood in multiple areas on each piece of equipment that pose a scratching hazard.

Plan of Correction: Our maintenance tech was here on Friday (November 1st) and we got approval for repairing and replacing any of the wood that needs replacing.
