Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on May 22, 2020 and concluded on May 22, 2020. The director was contacted by telephone to initiate the inspection. There were 11 children present and 12 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed two children?s records and two staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based on review of two children's records, the facility did not obtain all required information for one child's record.
Evidence:
1. The record of child #2, enrolled 03/20/2019, did not have the address listed for the second emergency contact. 2. The director acknowledged the record of child #2 did not contain an address for the second emergency contact.

Per the director: We will have the parent complete the missing information.

Description:
Based on review of documentation and interview, the facility did not obtain an annual fire inspection report from the appropriate fire official having jurisdiction.
Evidence:
1. The most recent documentation of a fire inspection was dated 02/07/2019, exceeding the annual requirement. 2. The director stated an annual fire inspection had not been conducted as of yet.
Plan of Correction:
Per the director: We will contact the Fire Marshall to schedule our annual fire inspection.
