Comments:
An unannounced monitoring inspection was conducted today from 11:00am-12:45pm. There were 5 children (11 points) directly supervised by 2 staff. The home, 2 staff records, 2 children?s records, 3 household member?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed coloring, gathering and participating in circle time, singing songs, reciting words in English and then Spanish and eating lunch. Lunch served today was chicken nuggets, fries, carrots, apples, and milk. After lunch, children washed hands and were playing freely. There was a supply of books, toys, and materials for the children. Areas of non-compliance are identified in the Violation Notice.

Based on observation, the first aid kit did not contain all required items.
Evidence: The first aid kit is missing a second triangular bandage.

I will make sure to have a second triangular bandage in the first aid kit.

Based on record review, the provider did not review with the parent the emergency contact information to ensure the information is correct annually.
