Comments:
An unannounced Monitoring Inspection was conducted today from 7:15am to 8:10am. There were 15 (school-agers) directly supervised by 3 staff. The physical plant, 3 staff records, 5 children?s records, 2 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play creativity project, presenting their work, and breakfast preparations. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Based on review, the facility did not ensure that the each physical examination and dates immunizations were received and shall be signed by a physician, their designee, or an official of a local health department.
Evidence: Reviewed 5 children's records and found that Child 4's physical examination was signed by the parent.

Will call parent today.
