Comments:
An unannounced monitoring inspection was conducted on 01/24/2023 between the hours of approximately 10:40 a.m. and 11:10 a.m. There were 0 children present with the Provider at the time of inspection. A complete inspection of the physical plant, children and caregiver records, and fire drill log were observed during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review, within seven business days of each child?s first day of attendance, the provider did not obtain from the parent verification of the identity and age of the child.
Evidence: The provider did not obtain verification of identity and age for Child #1.
Plan of Correction:
I will ask them for the birth certificate.
