Discussed with the provider: The next anticipated inspection will be the provider's renewal. The provider was reminded to submit all renewal paperwork to the licensing office via the online portal 45 days prior to the expiration of the license. The new renewal application, as well as a link to the online portal, can be found on the VDOE website.

An unannounced monitoring inspection was conducted on October 26, 2023 between the hours of approximately 10:40 a.m. and 11:15 a.m. A complete inspection of the physical plant, caregiver records, fire drill log, menu, attendance and emergency procedures were observed during this inspection.
