Comments:
An unannounced renewal inspection was conducted today from 11:00am-1:45pm. There were 90 children directly supervised by 17 staff. The physical plant, 14 staff records, 11 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in tummy time, reading books, transitioning inside from outdoor play, and eating lunch. Hand-washing procedures were also observed. Teachers were observed actively engaging with children in a positive and constructive manner There was an variety of age appropriate suppliesy for the children served by the center. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22VAC40-185-270-A
Description: Based on observation, one area of the center was not maintained in s safe condition.
Evidence:
In the toddler room a play kitchen sink was observed to be croken on the corner resulting in a sharp edge which could snag or cut skin.

Plan of Correction: We will repair the broken sink.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous materials were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. In the Infant 1 room an unlocked cabinet was observed to contain spray disinfectant.
2. In the Preschool 2 room an unlocked cabinet conained a bottle of spray cleaner, a bottle of bleach and water spray, and a chemical sponge.

Plan of Correction: We will ensure the cabinets are locked.

Standard #: 22VAC40-185-280-I
Description: Based on observation, art materials posing a hazard to children was used.
Evidence:
An art project in the Two's classroom was observed to contain small pom poms and foam sut out pieces which pose a choking hazard to children under 3.

Plan of Correction: We will not use these materials for projects in this classroom.

Description: Based on observation, two prescription medications were observed without the prescription label attached.
Evidence:
1. A medication for child 8 was observed without the prescription label.
2. A medication for child 9 was observed without the prescription label.

Plan of Correction: We will ask the parents to provide the medication box containing the missing label.
