Comments:
An unannounced monitoring inspection was initiated and concluded on 11/16/2022 in response to a serious injury report received by the Department of Education on 11/4/2022. The inspector was on site on 11/16/2022 from approximately 3:57 pm-4:26 pm. There were 22 children present, ranging in ages from five (5) to 10 years, with two (2) staff supervising. The inspector reviewed compliance in the areas of staffing and supervision and special care and emergencies. Interviews were conducted on site.

Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program.

Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Description: Based on review of documentation and interview, the center did not ensure that the required emergency supplies were available at the center.

Evidence:
1. On 11/4/2022, the Department of Education received a serious injury report that a child had sustained an injury that required medical attention on 11/3/2022. In the report, the center's representative wrote that the center was out of instant ice packs so they were unable to give ice to the injured child. On 11/16/2022, staff were interviewed on site. Staff #2 who was present on site the day of the injury confirmed that the staff were unable to locate and provide ice or a cooling agent. An ice pack or cooling agent is an emergency supply that is required to be available at the center.

Plan of Correction: Obtain at least 10 ice packs at all times
