Comments:
A non-mandated monitoring inspection was conducted on June 20, 2023 from 12:10 P.M.-2:00 P.M. There were four children present for a total of eight points with one caregiver. The inspector reviewed four children records, one caregiver record, and two household members records. The inspector reviewed and observed for compliance in the areas of administration, children records, physical plant, nutrition, programming, supervision, and special care and emergencies.
Information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice provided to the facility.

Based on review of children records, the provider failed to ensure that a copy of the physical examination was obtained within 30 days of enrollment.
Evidence: 1. Child #4's date of enrollment was 08/16/2022. There was not documentation of a physical examination on file. 2. The provider verified that there was not a copy of the physical examination on file.
Plan of Correction:
In the future any child that enrolls will have to have the physical examination paperwork before they can begin attendance.

Description:
Based on observation, the provider failed to ensure that the criteria on the health and safety checklist was met at the time of the monitoring inspection.
Evidence: 1. On 6/20/2023 the Licensing Inspector observed the provider change two diapers. The children did not wash their hands with soap and water after diapering. 2. The provider verified that the children did not wash their hands with soap and water after diapering.
Plan of Correction:
The provider was unaware of the children have to wash their hands after having their diaper changed. She will immediately put into practice that after every diaper change the children will wash their hands with soap and water.
