Comments:
A monitoring inspection was conducted on May 7, 2024 from 11:10 A.M.-12:45 P.M. There were four children present and one caregiver. The Inspector reviewed five children?s records, the provider, one caregiver, and three household member?s records for updates. The inspector reviewed and observed for compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on a review of caregiver records, the provider failed to ensure that all caregivers completed a sworn disclosure statement prior to the first day of employment.
Evidence: 1. Caregiver #2's date of hire was 02/12/2024. There was not a completed sworn disclosure statement in the record for Caregiver #2. 2. The provider verified that there was not a completed sworn disclosure statement in the record for Caregiver #2.
Plan of Correction:
Caregiver #2 will not be back until the Fall, but the provider will email a copy of the sworn disclosure statement for her to complete.

Description:
Based on review of pet vaccines, the provider failed to ensure that all rabies shots were kept up to date.
Evidence: 1. Pet #1's rabies shot expired on 04/06/2024. 2. The provider verified that a new rabies shot had not been obtained.
Plan of Correction:
The provider will obtain an appointment to get an updated rabies vaccination.
