Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone/email contacts, documents review, interviews and a tour of the program. A monitoring inspection was initiated on 8/3 and concluded on 8/5. The Provider was contacted by email and a virtual inspection was conducted. There were 10 children present, ranging in ages from 2 to 5 years, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 caregiver/household members records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on file review, 1 out of 2 children's records did not have all required information. Evidence 1. Child #1 does not have parent's work addresses, health insurance policy number or addresses for both emergency contacts in case the parents cannot be reached.

The Provider will ask the parent for this information.
