Comments:
An unannounced monitoring inspection was conducted on April 21 2022 between 10:45am and 12:00pm. There were 3 children in the direct care of the provider. The children were observed eating and washing their hands. The inspector reviewed child records, household records, and the physical space of the home. Citations are listed on the violation notice.

Based on record review, the provider did not maintain an up-to-date record of all children in care.
Evidence: Child #1 did not have a record on file. The provider stated that she provided the paperwork to the family, but they had not returned it yet.
