Comments:
A code compliance inspection was conducted on April 23, 2024. There were fifty children present with five staff supervising. Additional staff were on site for administrative duties. Children were observed during nap time. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of five staff records were reviewed. Violations found are noted on the violation notice.
Based on review, facility did not ensure 1 of 5 staff have documentation from a practicing physician that the individual is free from any disability which would prevent him from caring for children.
Evidence: The record for staff #2 has a most recent staff health form signed by a physician on 4/5/2022.
Plan of Correction:
Staff will get the staff health form completed by a physician.
