Comments:
The licensing inspector conducted an unannounced renewal inspection, at which breakfast time, personal hygiene,play groups, group time, activities, and outdoor play time were observed. Children were observed; and staff and children files and other documentation were reviewed. Fifteen (15) children, aged 2-4 years, were found to be in care with 4 staff, kitchen staff, and administrative staff. Previous violations were reviewed, and one (1) of the violations from the previous inspection had not been corrected. Please contact me if you need further assistance. Thank you for your cooperation. This inspection began at 8:30 am and concluded at 11:30 am.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on review of children records the facility failed to maintain compliance with this standard as required relating to proof of physical. Non-compliance with this standard was previously cited on 10/30/2019, and therefore is marked as a repeat violation.

Evidence:
Child record #1 (from previous inspection on 10/30/19) still did not have proof of a physical as required and the date of enrollment was 4/2019..
Child record #2, enrollment date 8/2019, did not have a proof of physical as required and the allowed 30 days to obtain one had lapsed.
This is a repeat violation.

Plan of Correction: Administrative staff made contact with the parents of the children and requested the physical form as soon as possible. In the future if the forms are not received prior to enrollment or within the allowed 30 days after enrollment, then care will be suspended until the information can be obtained.
