Comments:
An unannounced renewal inspection was conducted today with the provider, who was present with 9 children and a second caregiver. The age of the children ranged from 11 months to 5 years for a total of 18 points. The children and provider were preparing for lunch at the conclusion of the inspection. Lunch consisted of pizza rolls, banana, carrots and milk.
Based on a review of five children's files and an interview with the provider, it was determined that a immunization record was not obtained for one child before the child attended the family day home. Evidence: Child A enrolled on 2/14/22 and there was no immunization record on file.
Plan of Correction:
I will get the immunization record.
