Comments:
The licensing inspector conducted an unannounced monitoring inspection from 9:20am - 11:10am. There were eight toddlers in care with three staff. The children were observed playing with play dough, puzzles, going outside to play and preparing for lunch. Five children's records were reviewed. One violation from the previous inspection was repeated. Areas of non-compliance are identified on the violation notice. Based on record reviews, the center failed to obtain documentation of a physical examination.
Evidence:
1. The inspector reviewed five children's records. One of five records reviewed (child record #1) did not contain documentation of a physical exam and child's date of enrollment was 08/20/2018.
Plan of Correction:
The teacher will remind the parent again that a copy of the child's physical is needed for the child's record.
