On 8/20/2019, an unannounced monitoring inspection was attempted at approximately 3:25pm. Any violations are noted on the Violation Notice. If you have any questions, please contact me at shahana.green@dss.virginia.gov .
Based on observation, the licensee did not afford the department's representative reasonable opportunity to inspect all of the facility's buildings, books, and records. Evidence: 1. On 8/20/019, at approximately 3:25pm, Staff A denied the agent access into the family day home while there were children in care.
Plan of Correction:
The children's records have moved to downstairs.
