Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated on and 4/15/2021 concluded on 4/15/2021. The provider was contacted by telephone to initiate the inspection. There were 3 children present and 1 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records, 1 household member record and 1 staff record submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. Based on review of medication and interview, medication with an expired authorization form was not returned to the parent within 14 days of expiration.
Evidence: The medication authorization form for a prescription allergy medication for Child#3 expired on 9/30/2020. The medication was observed on-site during the inspection.

I have provided a new authorization form for the parents to take to their pediatrician.
