Standard #: 8VAC20-780-160-A-1
Description: Based on review of two staff records and interview, the center did not obtain documentation of a tuberculosis screening at the time of employment and prior to coming into contact with children.

Evidence:

The record of staff #2, hired 02/01/2022, did not contain documentation of a tuberculosis screening. The director stated that staff #2 started working with children on 02/01/2022 and had not submitted a tuberculosis screening.

Plan of Correction: Per the director: The staff member will schedule an appointment and have the tuberculosis screening completed within 7 days.

Description: Based on review of documentation, the center did not obtain an annual fire inspection report.

Evidence:

The most recent fire inspection documentation was dated 09/10/2020, exceeding the annual requirement.

Plan of Correction: Per the Director: A fire inspection has been scheduled for February 15, 2022.

Description: Based on review of documentation, the center did not obtain an annual approval from the health department.

Evidence:

Documentation of the most recent health inspection report was dated 09/16/2020, exceeding the annual requirement.

Plan of Correction: Per the director: I have scheduled the health department to conduct the inspection on February 15, 2022.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center did not maintain areas of the building in a clean, safe and operable condition.

Evidence:

1. The door leading into the kitchen from the preschool classroom did not have a door knob. The knob was not attached and exposed the four metal protruding prongs where the knob would attach.
2. An area of the wall, approximately five inches long, at the baseboard in the preschool classroom had peeling paint and exposed plaster from a crack.
3. The four seat feeding table in the infant room was wobbly and unstable when two infants were seated for lunch.
4. Three sleeping mats occupied by children were torn along the side exposing the interior foam.

Plan of Correction: Per the director: 1. A new doorknob was installed. 2. I will have the wall repaired. 3. I will tighten the screws on the table to secure it soundly. 4. The children do not usually sleep on the mats but rather the cots located on the second floor. Today the children slept on the first floor and used the mats while repairs on the upstairs restroom were being conducted. We will dispose of the mats and children will return to sleeping on the cots.

Standard #: 8VAC20-780-340-F
Description: Based on observation, the center did not ensure children under 10 years of age always shall be within actual sight and sound supervision of staff, except that staff need only be able to hear a child who is using the restroom.

Evidence:

1. At approximately 11:49 am, staff #1 left five children in the classroom and entered the kitchen to gather lunch. Staff #1 repeated this two additional times before opening the door between the kitchen and classroom to be able to see and hear the children.
2. At approximately 12:07 pm, staff #2 left the infant classroom to go to the kitchen leaving two infants unattended. A few minutes later staff #2 left the Infant classroom again to retrieve a vacuum cleaner.

Plan of Correction: Per the director: I will review supervision requirements with staff. Usually there is a third staff member in attendance to assist with coverage, however I had a staff out that day and I was out of the building as well for an appointment.

Standard #: 8VAC20-780-570-E
Description: Based on observation, the center did not ensure prepared infant formula was refrigerated, dated and labeled with the child's name.

Evidence:

Two prepared bottles that were not dated or labeled with the child's name were sitting next to each other on top of the refrigerator. There were two infants in care at the time of the observation.

Plan of Correction: Per the director: corrected. I will review with staff to label all bottles with the name of the child and date as they arrive to the program.
