Comments:
An unannounced mandated inspection was completed from 10:35am. until 11:55. There were 14 children present with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on record review, the center did not have all the required information in a child's record. Evidence: 1. Child #3 had not proof of ID and age in the file; it is required to be in the file within 7 days.
Plan of Correction:
Staff will get proof of age and id from the parent.
