Comments:
An unannounced monitoring inspection was conducted on April 9, 2024 between 9:00am and 11:35am. There were 11 children present with two (2) caregivers supervising within the supervision guidelines. The children were observed playing outside, eating breakfast, handwashing, toileting procedures, and playing with other age-appropriate toys and materials. Positive interactions were observed during this inspection. The inspector reviewed compliance in the areas of administration, required posted documents, physical plant, staffing and supervision, programing, evacuation drill logs, menu, first aid supplies, attendance, child and caregiver records, and emergency procedures. There was no medication on site. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-770-60-C-2
Description: Based on record review, the family day home did not obtain a central registry finding within 30 days of employment.
Evidence:
1. Staff #3 (DOH 3/1/24) did not have a central registry finding at the time of inspection.
2. Staff #4 (DOH 6/5/23) did not have a central registry finding at the time of inspection.

Plan of Correction: I submitted the paperwork. I did not realize it was for the portability and had been declined. I will submit the correct form today.

Standard #: 8VAC20-800-750-A
Description: Based on observation, the topical skin product was not labeled with the child's name.
Evidence:
1. The topical skin product (Aquaphor) for Child #4 was not labeled with the child's name.

Plan of Correction: We just received the cream yesterday and we forgot to write the child's name on it. We will be more careful.
Violation corrected during inspection.

Standard #: 8VAC20-800-90-A
Description: Based on record review, the provider did not obtain documentation that the child was adequately immunized before the first day of attending.
Evidence:
1. The record for Child #5 (DOE 3/18/24) did not have documentation of immunizations.

Plan of Correction: I will tell the parents today that they must bring the immunization and the physical for their child.
