Comments:
An unannounced inspection was conducted on 9/3/2019 based on a complaint received 8/29/2019. An inspection of the facility, children and staff records, accident reports and menu's were conducted with the Assistant Director and the Director.
Information gathered during this investigation supports the allegation of late and incomplete emergency procedures and substitutions to the menu that were not documented, therefore the complaint is determined to be valid.

Based on a review of staff records, the center failed to ensure that at least every two years from the date of the first initial screening or test or more frequently as recommended by a licensed physician, staff members shall obtain and submit the results of a follow-up tuberculosis screening.

Description:
Based on a review of the accident report and interviews with administration, the center failed to maintain a written record of children's serious and minor injuries in which entries are made the day of the occurrence and the record shall include documentation of staff and parent signatures.
Evidence: The injury occurred 8/23/2019 but parent's were not notified until 8/28/19, the Assistant Director received the accident report on 8/26/2019 and she did not complete it and send to parents until 8/28/19. The accident did not have staff or parent signatures.
