Comments:
A modification inspection was conducted today from 9:00 am until 12:00 pm to review the space converted from a preschool classroom to an infant classroom. The physical space, cribs, changing tables, evacuation routes, infant curriculum, evacuation plans and maps, emergency supplies and staff records were inspected. Based on a reivew of staff records the center failed to obtain at least every two years from the date of the first initial screening or testing or more frequently as recommenced by a licensed physician or local health department, staff members shall obtain and submit the results of a follow up tuberculosis screening as stated in subsection B of this section.
Evidence: Staff member #3 does not have documentation of a current tuberculosis screening or test result.
Plan of Correction:
Staff member was contacted 12/18/2019 and asked t submit a new tuberculosis test or screening results.

Description:
Based on observation, the center failed to ensure that electrical outlets shall have protective covers that are of a size that cannot be swallowed by children.
Evidence: The power strip in the infant classroom did not have protective covers on the unused outlets on the strip.

Description:
Based on a review of staff records, the center failed to obtain a copy of the results of a search of the child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided during the last five years.
Evidence: Staff #3 does not have documentation in the reocrod of a complete central registry or equivalnect report from another state. Staff member lives in Maryland.
