Comments:
An unannounced monitoring inspection was conducted today between 10:15 am and 1:30 pm. There were 34 children present with 7 direct supervision staff. The children were observed during playtime and lunch time. Lunch included hamburger (hummus for vegetarian offering) spinach salad,fruit and milk. The children were also observed going to the playground, during diaper changing and hand-washing. The physical plant, attendance logs, evacuation logs, children's records, staff records, emergency supplies and medications were inspected. Areas on non compliance were cited on the Violation Notice. Based on a review of children's records, the center failed to ensure to obtain and maintain documentation of required records for each child enrolled.
Evidence: 1 Child #1's records did not have documentation of the phone number of child's physician, 2.Child #2''s records did not have documentation of work phone number and place of employment of each parent.

Description:
Based on a review of evacuation log, the center failed to implement a monthly practice evacuation drill.
Evidence: The center did not have documentation of an evacuation drill for January 2020.
Plan of Correction:
I will remember to complete monthly evacuation drills and document them.
