Reminder: Each staff member needs to obtain 16 hours of training annually.
Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a stat of emergency health pandemic declared by the Governor of Virginia. A renewal inspection was initiated on May 11th, 202 and concluded on May 11th, 2020. The owner was contacted by telephone to initiate the inspection. There were no children present and no staff present. The inspector emailed the owner a list of items required to complete the inspection. The Inspector reviewed 2 children's records and 2 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. Based on a review of children's records, the center failed to maintain accurate up to date records for each child enrolled.
Evidence: Child #1's record does not contain documentation of work phone number and place of employment of each parent who has custody.
Plan of Correction:
I will get the information from parent and place in file.
